WellCare Foundation

Profile of WellCare Foundation, a Nonprofit Organization in Phoenix

By

WellCare Foundation is a nonprofit organization doing business in the Greater Phoenix area. I posed ten questions to a representative of WellCare Foundation in an effort to familiarize Phoenix area residents with the organization. Here are the responses I received.

What is the name of the organization?

WellCare Foundation. Mission: to provide free, continuous, integrated healthcare to single, working mothers and their children in need.

How long has the organization been operating in the Phoenix area?

Founded as a 501c3 in 1999.

Does the organization serve a particular geographic area?

Maricopa County

Who is most likely to benefit from services provided by the organization?

Single, working mothers and their children who are not on state assistance and cannot afford the cost of health insurance or needed healthcare. The entire community benefits because healthy moms miss fewer work days, and happy, healthy children miss fewer school days and are more alert.

Are there requirements or prerequisites for people to access your services?

If moms think they meet the above criteria, they can fill out an eligibility form on the WellCare Foundation website or call to be sure they qualify. Once eligibility is determined they will receive a free health assessment, including needed labs and x-rays, and an individualized plan will be implemented. Healthcare is provided through the generosity of many medical professionals throughout Maricopa County who donate their services to provide free healthcare.

Three things about WellCare that people might not know.

  1. The 2006 Community Census revealed that there are over 175,000 mothers and children in Maricopa County without health insurance. The WellCare Foundation is currently providing free healthcare to 350 families (a total of approximately 1,050 moms and children). They have the capacity to serve many more deserving families in need.
  2. One of WellCare’s community outreach goals is to establish more clinics in the east and west areas of the Valley. Clinic space is currently donated by Boys and Girls Club in Tempe, Save the Family in Mesa, and the main office and clinic is housed at 55 E. Lexington Avenue, Phoenix. The WellCare Foundation would like to establish clinics in Glendale or Peoria, in Gilbert and in northwest Phoenix. Increasing the number of clinics throughout Maricopa County will allow WellCare to serve more qualified and deserving single working mothers and their children.
  3. Among the integrated health services offered through WellCare are: Pediatrics, Internal Medicine, Counseling, Nutrition, Ophthalmology, Speech Therapy, Gynecology, Acupuncture, Osteopathic Medicine, Cardiology, Naturopathic Medicine, Dermatology, Chiropractic, Radiology, MRI and necessary lab tests. WellCare also offers a special program for mothers and children at risk for Type II Diabetes due to obesity and or predisposition due to family history.

What is the local address?

55 E. Lexington Avenue
Phoenix, AZ 85012

What is the local phone number?

602-263-7619

What is the URL of the local web site?

www.WellCareFoundation.org

Any other comments?

Everyone knows a WellCare mom. These moms could be your neighbor, a relative, friend or co-worker.

Each WellCare mom receives a “Pay It Forward” WellCare Bear. She is asked to give the bear to another mom who might also qualify to receive free, integrated healthcare through the WellCare Foundation. Please visit the WellCare Foundation website to find out how you can help to “Pay It Forward.”
